Acupuncture is a holistic practice. To provide the most accurate diagnosis and effective treatment, we need to understand the complete picture of your health—physically, mentally, and emotionally.

Our OPC form is comprehensive yet easy to use. Most questions are multiple-choice, but text boxes are also provided for you to add more specific details.

A vital part of our diagnostic process involves visual observation, particularly of the tongue. Therefore, where possible, we encourage you to upload a clear photo of your face and tongue with your form.

2.    Upload Your Photos: For a more precise diagnosis, please upload a recent, clear photo of your face and tongue.

 Tips for a useful diagnostic photo:

  • Use natural daylight if possible.

  • Do not apply any beauty filters.

  • Do not wear makeup.

  • Take the photo around the same time you fill out the questionnaire. 

3.    Submit the Form: Your practitioner will carefully review your submitted information prior to your scheduled appointment.
